“I Know I Need Help, but I Cannot Ask”: Exploring Psychological and Social Barriers to Mental Health Help-Seeking Among Young Adults

Abstract

Objective: This study aimed to explore the lived psychological and social barriers that prevent young adults in the United States from seeking mental health support despite recognizing that they need professional or interpersonal assistance.

Methods and Materials: This qualitative study employed a descriptive phenomenological orientation and included 26 young adults aged 18–29 years residing in the United States. Participants were recruited through purposive and maximum-variation sampling and were eligible if they had experienced psychological or emotional distress during the previous two years, recognized a need for support, and nevertheless delayed, avoided, or experienced substantial difficulty asking for help. Data were collected through individual semi-structured, in-depth interviews lasting approximately 45–75 minutes, supplemented by demographic information and interviewer field notes. Interviews explored perceived need, self-reliance, emotional disclosure, stigma, burden concerns, interpersonal experiences, and conditions facilitating help-seeking. Interviews were audio-recorded, transcribed verbatim, and analyzed using inductive reflexive thematic analysis. Coding, category development, constant comparison, negative-case examination, reflexive documentation, and iterative theme refinement were used to enhance analytical credibility and trustworthiness.

Findings: Analysis generated six overarching themes: pressure to manage distress independently, uncertainty about whether psychological problems were sufficiently serious to justify help, difficulty articulating internal experiences, fear that disclosure would negatively alter others’ perceptions, reluctance to burden significant others, and conditional willingness to seek assistance when sufficient relational safety was present. These themes indicated that help-seeking was shaped by interacting psychological and social processes rather than isolated barriers. Participants commonly moved through a recursive pathway involving recognition of need, internal legitimacy assessment, communication readiness, evaluation of anticipated social consequences, and a perceived balance between the benefits and costs of disclosure. When psychological and interpersonal costs remained high, help-seeking was delayed or avoided; when validation, trust, confidentiality, normalization, and explicit invitations to disclose reduced those costs, movement toward support became more likely.

Conclusion: Mental health help-seeking among young adults is best understood as a dynamic negotiation between recognized need and perceived psychological and social safety, suggesting that interventions should address not only mental health awareness but also self-reliance norms, legitimacy judgments, communication difficulties, stigma, burden concerns, and relational conditions that determine whether recognition of need is translated into action.
